Daily Living Support Assistance with Daily Life supports NDIS participants to live as independently as possible by providing help with everyday personal tasks and routines. This support can be delivered in your home or in the community and is tailored to your unique needs and preferences. Our trained and compassionate team at Reach Ability Support can assist with: Personal care (showering, dressing, grooming) Meal preparation and nutrition support Medication Assistance Household tasks (cleaning, laundry, basic home care) Assistance with shopping and appointments Help with morning and evening routines Promoting skill-building and independence in the comfort of your own home ​ Whether you require a few hours a week or daily support, we’re here to ensure you feel safe, respected, and supported in your day-to-day life.

Medium Term Accommodation (MTA), Short Ter Accommodation, Supported Independent Living (SIL) and Transitional Support Medium Term Accommodation (MTA) ​ MTA provides a safe, supported living option for NDIS participants who are waiting for permanent housing. It's designed as a temporary solution, typically up to 90 days, offering stable accommodation and daily support while longer-term arrangements are finalised. ​ Short Term Accommodation (STA) STA, including respite, allows participants to stay in a supported setting for a short period — giving both the participant and their carers a break. It includes accommodation, personal care, meals, and support with activities, and can be used for rest, recovery, or building independent living skills. ​ Supported Independent Living (SIL) SIL is for participants who need help with daily tasks while living in a shared or individual home. Support is tailored to each person and may include help with personal care, cooking, cleaning, medication, and community participation — empowering individuals to live more independently. ​ Transitional Support Transitional Support helps participants adjust when moving between accommodation types or after hospital discharge, interstate transitions, or other life changes. It focuses on short-term, goal-oriented support that builds independence and prepares the individual for a more permanent living arrangement.

Level 2 & Level 3 Specialised Support Coordination ​Level 2 Support Coordination helps NDIS participants build the skills and confidence needed to understand, implement, and manage their NDIS plans. Our experienced Support Coordinators work closely with each participant to connect them with the right providers, coordinate services, and ensure they’re getting the most out of their funding. ​ ​​ Specialist Support Coordination (Level 3) is a higher level of coordination provided to NDIS participants who face significant and complex challenges in their lives. This may include people with high support needs, multiple or unstable support arrangements, or involvement in guardianship, justice, or housing systems. At this level, a Specialist Support Coordinator takes a hands-on, intensive approach to help stabilise the participant’s situation and build a safe, consistent support environment. ​ Our team is experienced with Complex Hospital Transitions

Consulting Services At Reach Ability Support, we offer specialist consulting services for individuals, families, providers, and professionals navigating complex NDIS-related matters. ​ Our team brings deep expertise in NDIS service delivery, transitional pathways, and complex case management — including matters involving the Administrative Appeals Tribunal (AAT), escalating support needs, and crisis intervention. ​ We are well connected with a broad network of referrers and support systems, including: Advocates and Local Area Coordinators Justice and corrective systems Homelessness and crisis accommodation services Allied health and clinical support networks Whether you're navigating a challenging transition, managing a high-risk situation, or seeking guidance on complex NDIS planning, our team provides strategic, person-centred advice to support positive outcomes.
